Attention Parents/Guardians of Fifth Grade Students

As of September 2008, students born on or after January 1,1997 who are enrolled in grade 6, or transferring into a New Jersey school from another state or country, are required to receive a booster dose of the diphtheria, tetanus, and pertussis (Tdap) vaccine, as well as one dose of Meningococcal vaccine.  These two vaccines are in addition to other vaccinations required by the New Jersey Department of Health for attendance in public school.
